AI Agent Production Approval Template
A successful pilot is not a production approval. Before an agent receives customer data or write access, someone must record what is permitted, why, and who can stop it.
This copy-ready memo is for procurement, security and third-party risk teams. It turns vendor evidence into a bounded internal decision, without treating a questionnaire or independent report as permission to deploy.
Use This After the Evidence Review
Start with the vendor security questionnaire to request evidence. Use this memo after reviewing the answers, not instead of reviewing them.
Record the exact product, deployment mode and tool configuration. Approval for a read-only sandbox does not cover a production agent with database writes or payment authority.
Select and copy the text below into your internal approval system. Replace every bracketed field. Keep sensitive evidence in your restricted repository, not in a public report or page.

Write Conditions That Can Be Closed
“The vendor should improve security” is not a release condition. A condition needs a named owner, observable evidence and an access restriction until it is closed.
Illustrative condition: before enabling production database access, the security reviewer must verify read-only permissions with a dedicated identity. Write access remains disabled until separately approved.
Illustrative recovery condition: before the agent handles business-critical work, the operations owner records a successful restore check from a backup the organisation controls.
These are examples, not findings about a vendor. Your controls must match the workflow, threat model and evaluation you actually performed.
Attach the Report, Keep the Decision Yours
An independent verdict and an internal deployment decision are different records. Attach the report by title, date and link; preserve its limitations and remediations.
Do not translate CONDITIONAL GO STRICT into unrestricted approval. Identify which conditions are prerequisites, which require local verification, and which residual risks your authorised approver accepts.
The public Manus report shows recovery and restricted-authority conditions. The public Replit Agent report shows why environment boundaries belong in the decision record.
Both are based on publicly available information only. Neither verifies your configuration or replaces private contractual review, authorised testing, or a restore check.
Source Basis and Limitations
This template is TrustworthAgent’s practical synthesis, not a prescribed regulatory form. It does not establish compliance, transfer liability, or provide legal advice.
The NIST AI Risk Management Framework 1.0 supports documented responsibilities, deployment-risk decisions and residual risk through GOVERN 2.1, GOVERN 2.3 and MANAGE 1.4.
NIST GOVERN 1.5 supports periodic review. MANAGE 2.4 and MANAGE 4.1 address deactivation, recovery and change management. NIST does not prescribe this memo or a universal approval duration.
The OWASP Top 10 for Agentic Applications 2026 informs the access and recovery fields: ASI02 Tool Misuse, ASI03 Identity and Privilege Abuse, and ASI10 Rogue Agents.

Frequently Asked Questions
Does an independent report approve our deployment?
No. A report provides a dated assessment and an independent verdict. Your authorised approver owns the deployment decision, including any accepted residual risk and the actual access boundary.
Can public documentation close every condition?
No. Public material can support a desk-based finding, but it cannot verify your configured permissions, private contractual terms, or recovery procedure. Keep those conditions open until the relevant evidence exists.
How long should approval last?
Choose a review interval using your risk policy, deployment exposure and change rate. The sources below do not prescribe a universal approval duration. Material changes should trigger review before expanded access.
